JOB SUMMARYPopulation Health is responsible for planning, directing, and managing population health activities for the health plan of Baylor Scott and White Health (BSWH). Leads the strategy, development, oversight, and execution of the health plan population health programs, which may also include disease management, vendor management, clinical analytics reporting and digital health strategy for the health plan.
