The position described is that of a Patient Aligned Care Team (PACT) Registered Nurse Care Manager in the Nursing-Primary Care (RN) Service Line for the Pike Clinic. The RN must possess a broad-based clinical knowledge to effectively implement health promotion, prevention, management of acute and chronic illness at the Pike County VA Clinic.
