The Community Health Worker (CHW) supports providers and the social work team through an integrated approach to care management and community outreach. This is a grant funded position from June 2022-June 2023 focused on COVIE vaccination. As a priority, CHW activity acts to promote, maintain, and improve the health of patients and their families, provide social support and informal counseling and advocate for individuals and community health needs.
