DescriptionThe Office of Population Health and Accountable Care (OPHAC) is focused on leading and facilitating the framework to improve population health outcomes across UCLA Health’s Alternative Payment Model (APM) populations across the care continuum. As APMs evaluate outcomes regardless of where the care was rendered, OPHAC initiatives are targeted at improving population-based outcomes for care at UCLA Health and outside of UCLA Health (e.g. post-acute care settings, pharmacies).
